Scheduling Model

The BCRP uses a 6-week block schedule divided into three 2-week segments designated A, B, and C blocks (sometimes referred to as a “4+2” model). Most A and B blocks are inpatient rotations with more intensive schedules, while C blocks are dedicated to outpatient experiences with lighter hours. During every C block, residents attend continuity clinics for at least two full days (and sometimes more), ensuring consistent time with their primary care patient panels every six weeks.

Each 6-week block may be divided into the following 2-week segments:

Importantly, the BCRP has eliminated all 24-hour shifts during residency. Instead, the program uses a night-float system to support wellness and provide sustainable, team-based overnight coverage.

Night Coverage at Our Hospitals

Overnight care is provided by dedicated night teams consisting of a supervising resident and an intern. Each night rotation spans two weeks, promoting continuity among team members and ensuring a smoother experience for both patients and residents. Residents on all night shifts have access to dedicated on-call rooms and receive meal allowances to support their well-being during these rotations.

PHM A/B/C Night Team: This team cares for all patients already admitted to the BCH Pediatric Hospital Medicine A, B, and C services.

BCH Heme/Rheum/Pulm Night Team: This team cares for all patients admitted to the BCH Hematology, Rheumatology, and Pulmonology Services. This team also admits any new patients overnight.

BMC Wards Night Team: This team cares for all patients admitted to the BMC Pediatric Ward and also admits any new patients overnight.

Admit Night Team: A recently established team responsible for admitting patients overnight from the BCH Emergency Department to the PHM A, B, and C teams. This structure allows the PHM A/B/C Night Team to focus on ongoing care for already admitted patients.

Floating Hospital Medicine Night Team: This flexible team provides cross-coverage across three different night services: PHM A/B/C Nights, PHM Admit Nights, and BMC Wards Nights. The team helps ensure seamless coverage and support for all primary night teams.
